You can fill in the form digitally straight away. Then print the completed document and add your signature. Send the form to GBLT, PO Box 1098, 8001 BB Zwolle.
Do you not have a tax assessment number? Then you can still use the consent form. We will ensure that we debit all your tax assessments in instalments.
Please note that processing the consent form takes at least two weeks. We may only debit your account after your consent has been processed. GBLT normally debits around the 26th day of the month.
